REGISTRATION FORM
Northeast Region institutions are eligible to attend the
NCAA Division II Leadership Academy February 15-17, 2008, in Philadelphia!
FINAL CHECKLIST:
Selected student-athletes and coach/administrator/faculty participants meet all requirements.
Will obtain signatures of all participants and director of athletics and submit to NCAA.
(Required forms to follow completion of registration process.)
All registrations will be submitted not later than January 7, 2008.
