Championships Forms

Most championships forms can be located by going to http://www2.ncaa.org/sports/index.html. After selecting a sport, various forms for that specific championship are available (e.g., AQ form, travel reimbursement, host information, facility evaluation).

NAIA/NCAA Joint Declaration Form (not available online)	Championships (Tina Krah)	This form is to be completed/signed by the institution AD and CEO and is to inform the two associations of intentions relative to participation in national championship competition during the upcoming academic year This form is not available online, form will be mailed to institution	Division II - September 15 Division III - August 15
