Division III Insurance Certification


The following information guide provides general information on the new legislation requiring certification of basic accident insurance coverage.  This guide is intended to explain the legislation, provide information on how to comply with the legislation, answer common questions, and provide sample forms that can be used as part of the compliance process.  The following information and materials are provided in this information guide:  

CERTIFICATION GUIDE

Overview of Legislation-  Includes new legislation reference, background information, intent of legislation and frequently asked questions.

Certification Flowchart-  A flowchart to help guide you through the compliance and decision-making process.

Filling in the Gaps-  A summary of compliance options to address scenarios where current practices do not meet legislation requirements.

Requiring Evidence of Insurance-  Suggestions on securing and reviewing evidence of insurance from parents/student-athletes.


SAMPLE FORM PACKAGES 1-  Examples of forms that may be of assistance for institutions that do not provide accident medical coverage (or a similar insurance-type program) and require student-athletes/parents to provide evidence of insurance coverage.

SAMPLE FORM PACKAGE 2-  Examples of forms that may be of assistance for institutions that provide insurance coverage, but want to notify student-athletes/parents of potential uninsured expenses and/or want to secure information about the type of other insurance student-athletes have.

SAMPLE CERTIFICATION OF COMPLIANCE FOR INSTITUTIONS-  This form can be completed and kept on file in the office of the director of athletics.
